Please note. If the AMDS membership is being purchased for another person and not for yourself, or you are using another persons PayPal account to pay for membership, then do untick the Delivery address box.

Then provide the actual name that is to appear on the membership card, and the address of that individual when completing your online payment. Otherwise the card will be made out in the name of the payee.
